Screenings will be done March through April 2012 at the PAT office at Field School, 302 Smith Avenue.
Appointment times are limited, therefore it is very important to keep your child's scheduled appointment, unless ill.
Eligibility Requirements:
Preschool: Child must be four years old BEFORE August 1, 2012
Kindergarten: Child must be five years old BEFORE August 1, 2012
Please Bring:
- Proof of residency (ex: utility bill or rent receipt)
- State issued birth certificate (or hospital certificate)
- Immunization records
- Social Security card (optional)
- Medicaid card (if applicable)
