End of the Year Information 2012

We are busy wrapping up this year and are looking forward to a GREAT year next year.

The Parents As Teachers office will be closed from May 18-August 6, 2012.

Kindergarten Screening and Enrollment will resume on August 6-9, 2012 at the Parents As Teachers office located in Field School – 302 Smith Ave.

Screenings will begin at 8:00AM and will be on a first come, first serve basis.  NO appointments will be made. 

ELIGIBILITY REQUIREMENTS:

Kindergarten:  Child must be five years old before August 1, 2012.

Please Bring:

• Proof of Residency (water bill, electric bill)
• STATE ISSUED Birth Certificate (or hospital certificate)
• Updated Immunization Records
• Social Security Card (Optional)
• Medicaid Card (If Applicable)

Please note that Preschool screenings have been completed for the school year 12-13.
